Below you will find useful information about our practice and several forms you can download for your convenience. 

Electronic Medical Records and Patient Portal

As a patient of The Orthopedic Center, you will have access to our secure Patient Portal at any time of the day. The portal allows new patients to register and complete the registration information online. As an established patient, the Patient Portal will allow you have access to part of your medical records. The portal will also give you the option to request information and find educational material. 

In order to access our online registration services, please click the Patient Portal button or link. The portal opens on a separate page and will give you the necessary prompts to complete your registration. 

Alternatively you can download the patient registration forms by clicking the icons below in PDF or Word formats. Once downloaded, print them at home complete the required information and bring the forms with you to your appointment.

Medical Records

  • Bring all records related to your present orthopedic condition

Diagnostic Studies

  • Bring all pertinent studies including X-rays, MRI's, CT scans, Bone scans, EMG's and NCV's as well as recent lab work. Also bring a copy of the official reading or interpretation of the study.

Medications

  • Bring a list of all the current medications you are taking. Include a list of all over-the-counter drugs, supplements and vitamins you are taking.

  • Bring a complete list of all know drug allergies.

Insurance and Identification are required

  • Bring your insurance card(s)

  • Bring your Driver's License

  • Co-pay/co-insurance payment is required at the time of service

Be Prepared to Discuss Your Condition

  • What precipitated the condition?

  • Is it getting worse?

  • How does it limit your normal daily routine?

  • Have you had any treatment including therapy, medications or surgery?
